New Patient Info

If you are a new patient we ask that you fill out an intake form.  Please click on the appropriate form above, fill it out, and bring it with you to your first appointment.  If you do not have access to a printer, you can come 15  minutes before your appointment time to fill out the form.

 

Your first consultation will be for 1 hour. This will ensure adequate time to begin a treatment program for you and to address the initial reasons for your visit. Treatment protocols vary from individual to individual even if two people express very similar symptoms.
